The Indiana Medical Error Reporting System (MERS) final report summarizes 134 reportable events in hospitals and outpatient surgery centers in 2018. Eight (8) of 134 events occurred at ambulatory surgery centers. Adverse Events such as leaving a foreign object inside a patient after surgery, operating on the wrong body part, (remarkably, these avoidable surgical errors still happen) or Stage 3 or 4 pressure ulcers (bedsores) acquired after admission, are based on NQF’s list of 28 serious, largely preventable adverse events.
