Massachusetts 2019 Serious Reportable Events shows surgical mistakes, pressure ulcers (bedsores), or serious injury or death after a fall or medication error, and other mishaps in MA acute care hospitals, non-acute hospitals and ambulatory surgery centers. Wrong site surgery (e.g. operating on the wrong body part) occurred during 2019 at Massachusetts General Hospital (9 incidents), 23 other hospitals, and 2 ambulatory surgery centers. Of the 497 Stage 3 & 4 pressure ulcers on acute care patients in 2019, 109 occurred at Brigham & Women’s Hospital. In non-acute hospitals, more than half of the reportable events were serious injury or death after a fall. Hebrew Rehabilitation Center (both locations) and Tewksbury Hospital each reported more than 15 serious falls. Excel files only. From Mass.gov Health and Human Services, November 2020.
