Report of adverse health events that happened in Minnesota hospitals, ambulatory surgery centers and behavioral health hospitals. Examples of these Never Events are pressure ulcers (bedsores) that resulted in a patient’s death or serious injury; falls; a foreign object left in during surgery, or operating on the wrong patient or wrong body part. They were reported between October 2020 and Oct. 2021, during the Covid-19 pandemic. There were a number of serious injuries and deaths. For the past two years, MN reports only for each facility and doesn’t tally the total for the state. Minnesota law requires the events to be reported. View facility-specific data in the interactive database, one hospital at a time. Adverse Events in the 2022 report came from close to 80 hospitals and ambulatory surgery centers. Report by MN Department of Health August 2022.
