No nursing homes were rated C or the lowest D rating. Twelve nursing homes were not yet rated by the state.
Find CA hospitals and long term care facilities (nursing homes) with 2022 penalties for violations or deficiencies that represent an immediate jeopardy to the health and safety of a patient or resident. The annual reports have now been replaced by the State Enforcement Actions Dashboard. Amounts of penalties (which used to be up to $125,000 per incident) are listed by name for 2022 and back to 2015. However, you MUST know the facility license number in order to see the fines. Otherwise, all the facilities are mingled together. In addition to hospitals and nursing homes, penalties for medical information breaches are identified. Almost $5 million in penalties have been given for medical information breaches since 2015. Total amounts for all penalties in 2021 were about $9.5 million. Most of the penalties were given to long term care facilities. Incident summaries do not appear to be available at this site. These facilities were recognized for a 3-year period. Their daily rates ranged from $224 (Avon Park) to $450 (Naples). Once you find a facility you’re interested in, you can compare family satisfaction survey results, quality scores and average cost per day prices. Dates are not always clear for the measures or prices, so it’s worth checking the technical report to find the dates. We found that the posted prices are from calendar 2019, when we checked in early May 2022. Federal inspection results are included, as well as the percent of staff who got the flu shot. There were 16 top-rated 5-star homes as of the October 6, 2022 update. Of those 16, nearly all scored well on quality measures (at least 3 stars), and had good ratings from both residents and families. CMS lists nursing homes in any state and District of Columbia that have a history of serious quality problems. These Special Focus Facilities (SFF) have been officially entered into the federal government’s SFF program to monitor care. Homes on the list may have a history of “yo-yo” compliance, with a good survey followed by substandard quality on the next, thus unable to sustain good care. Some nursing homes are listed as Showing Improvement or having graduated out of the program. In addition, CMS now reports hundreds of nursing homes (nearly 7 pages) that are candidates for the SFF program, based on quality issues over the prior 3 years. Up to 30 nursing homes per state are on the Special Focus Facility Candidate list (Table F). These nursing homes are likely just as troubled as those in the SFF program. CNA Abuse List. In North Dakota publishes a list of CNAs (nursing assistants) that may pose a caregiving threat to you or your loved ones. You can check to see if a certified nurse aide has been on the validated abuse list in the last 25+ years in ND. Six types of abuse are listed: mental, neglect, physical, sexual, theft and verbal abuse. State government list, updated in 2022.
