CalQualityCare.org provides this ratings site for California Nursing Homes, Home Health and Hospice organizations. Nursing facilities are rated on staffing, quality, inspections, complaints, cost and finance measures. Map shows by color which nursing homes are Superior, or less (down to Poor). Average charges for 2016 or older are shown. Home Health and Hospice are rated on quality measures. This site also serves as a directory for residential care (assisted living) and other senior care programs. The site was originally developed with the CA HealthCare Foundation, a nonprofit philanthropy. While it was once one of the best comparison sites available for California nursing homes, the data have not been updated since 2017 (using 2016 data). Now the site is managed alone by UC San Francisco.

However, an upside to this site is that nursing home room rates are in the detailed reports.

The IL Dept. of Public Health assembles a report of nursing home facilities which have been determined to be in violation of the Nursing Home Care Act. Facilities in Chicago and statewide. 2018 (and earlier) reports are available, plus outline of circumstances leading to as much as $150,000 fines for some violations. Reports are updated every quarter in 2019.

The Greater Detroit Area Health Council provides a 7-county health care performance report at MyCareCompare.org. Compare MI hospitals (side-by-side) for childbirth, COPD, emergency department (ER), heart attack, heart failure, cardiac surgery, hip or knee replacements, imaging, infections, patient safety, pneumonia, surgical infection prevention and overall patient satisfaction. See mortality ratings (survival rate) for some conditions. Same info as listed at the federal HospitalCompare site, except mycarecompare might not be updated as often, so check the federal site. The advantage of mycarecompare is that consumers can see Above Average and Below Average comparisons very quickly, for the selected hospitals they want to look at, and all health conditions at once. Very easy to use. Physician group and nursing home results are at this site. GDAHC hospitals are in Detroit, Ann Arbor, Pontiac and counties: Wayne, Washtenaw, Livingston, Macomb, Monroe, Oakland, and St. Clair. GDAHC was an early leader in quality transparency. NC Division of Health Service Regulation lists fines to long term care nursing facilities and adult care homes (senior and assisted living, rest homes, etc), showing date, amount and the type of violation during each year 2015 to 2019. We saw penalties over $125,000, which can signify major problems in North Carolina nursing homes in the past 3 years.

Click on the date for the annual survey findings, or an inspection survey that was done because of a complaint. Survey reports can be difficult to read, but they may provide a lot of detailed and useful information for consumers. Up to date with at least 3 years of reports through 2019

District of Columbia Health Department provides inspection reports online for their health care facilities, including nursing homes, assisted living, home care agencies, and more. Intermediate Care Facilities Division has a lot of the types of providers you might be interested in. Click on Survey Reports. Fairly easy to use, although inspection reports are rarely very easy to read, and may be older than you would like (e.g. 2014 or older). Nursing home reports may be more recent (e.g. to 2016). It may be difficult to find a survey report online from 2018 or 2019. Still, this Washington DC website is worth checking.

CMS lists nursing homes in any state and District of Columbia that have a history of serious quality problems. These Special Focus Facilities (SFF) have been officially entered into the federal government’s SFF program to monitor care. Homes on the list may have a history of “yo-yo” compliance, with a good survey followed by substandard quality on the next, thus unable to sustain good care. Some nursing homes are listed as Showing Improvement or having graduated out of the program. In addition, CMS has now added hundreds of nursing homes that are candidates for the SFF program, based on quality issues over the prior 3 years.
